It seems that no day passes by without reading an article in a Saudi website or newspaper about a medical error, some of which cause significant morbidity and sometimes death. We are in no way unique. Medical errors happen everywhere, but we have to learn from our mistakes. There is no excuse for poor quality work. There is no excuse for a serious medical error happening twice.
It bothers me a lot that there is no transparency at all when dealing with medical errors. As a transfusion medicine physician, I need to know what authorities did to investigate and correct the errors that contributed to the transfusion of HIV positive blood earlier this year, for example. The only information released was that a number of people were fired. Unless the system changes, something like this WILL happen again.
